Wesley Health Care Center has earned its reputation for excellence by providing superior long-term nursing care with a personal touch for over 35 years.

Health care services at Wesley include skilled nursing for the aging adult who has significant chronic medical needs and adults of other ages who require daily medical attention because of their chronic illnesses.  Wesley also offers a hard-to-find residential setting that can meet the light care needs of less seriously ill geriatric residents.  A specialized program for those requiring care and supervision for Alzheimer’s disease and dementia completes Wesley Health Care Center’s full spectrum of long-term care services.

Long-Term Care Programs

Care for the Aging Adult
Wesley Health Care Center has committed many of our resources to providing skilled nursing care to the aging adult who has significant chronic medical needs. We recognize that quality care is all about creating fulfilling relationships between people, so we nurture the trust and respect that evolves between residents and their caregivers.

Care for the Chronically ill Adult
Some of our residents are adults who have suffered an illness or injury resulting in the need for chronic care at an age usually associated with active living. We understand the unique challenges facing people who are adjusting to new ways of living and make their lives as rich and rewarding as possible while caring for their needs with respect and compassion.

Light Care Geriatrics
Wesley Health Care Center is one of few communities that offer a caring residence with a special program for older people whose day-to-day need for support is very real, but not as medically intense.

Alzheimer’s and Memory Support
Persons suffering from Alzheimer’s disease or dementia, the mind disorders that impair memory and perception and diminish life skills, require specialized care.  Wesley Health Care Center offers a long-term care unit specially designed to care for individuals who must deal with the two early stages of Alzheimer’s and chronic dementia. We provide a therapeutic environment that supports these residents with proactive programs and activities designed to keep them stimulated and alert in all aspects of life, as their ability allows.

  • Hospice care is offered as an added dimension to traditional care at Wesley Health Care Center in partnership with The Community Hospice of Saratoga.  The Community Hospice can be reached at (518) 581-0800.

  • Wesley has an enterostomal therapist on staff and state-of-the-art procedures and products to help residents who have hard-to-heal wounds.
